Traditional Membership:
Traditional Memberships automatically renew each month.
- Membership dues are billed monthly to your bank/checking account, credit or debit card on file.
- The Sentara RMH Wellness Center does not participate in Silver Sneakers or Silver and Fit.
- A minimum of 30 days advanced notice is required to cancel this type of membership.
What to bring with you when signing up:
- Valid ID (i.e. state-issued ID or drivers license, passport)
- Check, Cash or Credit/Debit Card for first payment of Enrollment fee and Pro-rated dues
To set up your monthly automatic payment, we accept the following: CHECKING/SAVINGS ACCOUNT (ACH) or VISA, MC, DISC CREDIT/DEBIT CARD.
Debit Card: VISA/MC/DISC
Credit Card: VISA/MC/DISC. Please note: A 3% Service Fee is added to payments made via credit card.
Checking/Savings ACH: No added service fee, but please note there is a $25 return fee charged if transaction is returned from the bank for any reason.
Individual:
One person age 16 and older.
Must provide valid ID.
Enrollment Fee: $175

Couple:
Two people living as a couple and residing at the same address. Must provide valid ID and proof of same address upon enrollment.

Family:
A couple and up to (2) dependent children through age 25. Dependents age 18 and older must provide valid ID upon enrollment.

One Parent:

Senior:
A Senior Membership is an Individual membership for a person age 62 or older. Must provide valid ID upon enrollment.

Senior Couple:
Two people living as a couple and residing at the same address. One person must be age 62 or over. Must provide valid ID and proof of same address upon enrollment.
